PROCEDURE TO OBTAIN A BUSINESS LICENSE

1. Fill out application form

A. Page 1 - all lines are to be complete

1. an application form for a state tax number will be provided, this number must be on the application before it will be accepted - if applicable

a. A business dealing only in service with no product sales is only required to write "no product sales" in the state tax number section

2. your social security number will take the place of the federal tax number if you do not have one

3. email address is optional

B. Page 2 - all applicable areas must be completed with copies of all certifications and permits

1. Contractor License - you will need a current Utah State contractor license, attaching a copy of it to the application and upon renewal every year

a. A contractor license is not required of a handyman business - job total can only be $1,000.00 without a contractor license.

2. Day Care - Adult and Child - you must show proof of applying for a state license and bring in a copy when received form the state and on renewal every year.

3. Animal Related - you must have a Regulatory Permit from the Carbon County Animal Shelter and attach a copy to the application and upon renewal every year

4. Food Related - you must attach a copy of your current food handlers permit and upon renewal every year

5. State Liquor License - you must first have a state liquor license before even applying - a beer license is different

6. Transportation Business - you must give a copy of a current insurance policy and upon renewal each year

7. Recreational Business - you must give a copy of your liability insurance and have ambulance director sign off - if this is for a one time event at the county fairgrounds, you must apply on the event license form.

8. Motels, Hotels, Mobile Home Parks - list all rooms and/or pads used and unused - you will be charged $5.00 per space

C. Page 3 will be completed by respective department

1. Animal Control - only if animal related business

2. Planning Department - all applications must be approved by this department at a fee of $15.00, before returning it to the clerk’s office.

3. Clerk’s Office - after application is completed return it to this office and pay your fee. A fee schedule is located in the county ordinance and will be prorated on a four quarter year.

4. You must appear at a County Commission meeting for final commission approval. The meetings are the first and third Wednesday of each month at 6 pm. This completed application must be into the County Clerk’s office by 5 pm that evening in order to attend the meeting.
